Covers all of your primary care needs, including annual physical and labs, repeated as necessary, in addition to 24 hour/7day a week access to telehealth and home visits at no additional charge, and care personalized to meet your health care needs.

Men’s Health Plan
$180 per month

Covers all primary care needs, including annual physical and quarterly labs with testosterone therapy for those requiring it. Telehealth is included in this plan. Home visits for quarterly labs are also included. Additional home visits will be charged at $100/visit.
